(a) Fracture of the skull, spine or pelvis.
(b) Fracture of any bone:
In the hand, except for the bone in the wrist or hand.
In the leg, except for the bone in the ankle or foot.
(c) Amputation of an arm or leg.
d) loss of vision.
(e) any other deterioration in the physical condition of a person which results in personal injury:
Admission to the hospital as inpatient for more than 24 hours.
If at sea, were limited to a bed on board, and this would result in admission to the hospital for more than 24 hours as inpatient if the vessel was in port.
ACCIDENTS (OTHERS) AND DANGEROUS ACCURATORS
The foreman or the Master as per good sound of Seajobs must also report any other accidents or
dangerous cases. Such reports should be sent to Pte Apex Ship Pte Management
Limited to 24 hours using the correct form. The company will report this
relevant authorities. See the Emergency Response Plans Guide for more information.
REPORTED DANGER
The following are examples of hazards that a company must have.
reported
(a) The collapse or overturning of any hoist, hoist, crane, derailment, inversion, ramp, mobile access platform, access equipment, mounting or boom seat or breakage of any cargo or part thereof.
(b) Explosion, collapse or rupture of any closed vessel, including a boiler or boiler tube containing gas (including air), liquid or vapor at a pressure exceeding atmospheric pressure.
(c) Any electrical short circuit or overload that results in a fire or explosion.
(d) Sudden uncontrolled release of flammable liquid, combustible gas or combustible liquid above the boiling point from any system, installation or pipeline.
(e) the uncontrolled release or release of any harmful substance or agent.
(f) any of the following in relation to any piping, valve or any piping system in the vessel:
Rupture, explosion or collapse of a pipeline or any part thereof, excluding minor leaks in pipes containing harmful substances.
Accidental ignition of any on the pipeline or any that was in the pipeline immediately before its ignition.
(g) Any contact with bulk asbestos fiber, except when wearing full protective clothing.
(h) any collapse or significant movement of cargo.
(i) Failure or collapse of any hatch cover or wire or hatch cover mechanism.
(j) Any fall overboard.
(k) Tow rope disconnection.
